County to the present. |

at the County Health Department where the child was born or at the
Ohio Department

Columbus, Ohio 43215 Phone: (614) 466-2531. You need an appointment to
search their indexes. Records found are NOT retrieved on the day
requested. Give 3-6 weeks for documents. |

Death Records and their indexes from 1908-1953 are found at the
Ohio Historical Society.
The Death Records Index from 1913-1944 is available on the
Ohio Historical Society's
Website. You can order death certificates from OHS or go there
yourself to copy the documents. |

Death Records and their Indexes from 1954 to the present are found at
Ohio Department
of Vital Records Columbus, Ohio. You need an appointment to
search their indexes. Records found are NOT retrieved on the day
requested, nor can you look at the actual records while you are there.
It takes 3-6 weeks to receive ordered documents. |

Death Records are held at the County Level at that county's Department of
Health or Vital Records. Contact that County for hours and copy
charges. |
